Registering
To register at this Practice, please complete the following registration form online. If you have any problems completing an Online Form, Please call into Reception between the hours of 1:00 pm and 6.00pm.
Registering at Benchill Medical Practice
Everyone in the UK has the right to free care from a GP. If you don’t have proof of ID or address, you can still register. Immigration status or nationality does not matter. Reception won’t ask for immigration documents and won’t share your information with the Home Office unless serious crime is involved
When you register with the practice you will need to attend an appointment with the Healthcare Assistant. In this appointment we will gather any relevant medical information. Your medical records will arrive to the practice usually within 6-8 weeks.
Named Accountable GP
All patients registered at Benchill Medical Practice are allocated a Named Accountable GP.
The Named Accountable GP works with relevant health and social care professionals to deliver a care packages. They ensure that patients have access to services when required.
Please contact the practice if you wish to know who this is. If you have a preference on which GP this is, we will make reasonable efforts to accommodate this request.
This does not prevent you from seeing any GP of your choice in the practice. You are unlikely to see any notable change in the way care is delivered to you by us
